1. GENERAL

Pursuant to Articles 11 and 13 of the Law on the Protection of Personal Data No. 6698 (“LPPD”), this Application Form has been prepared by İstanbul Ekspres Nakliyat ve Ticaret A.Ş. or the “Company” in its capacity as data controller in order to evaluate and resolve applications to be made by you, the data subjects, swiftly, effectively, and comprehensively.

2. METHOD OF APPLICATION

Under Articles 11 and 13 of the LPPD, you may submit your requests to our Company, as the data controller, regarding the implementation of the LPPD in writing by completing this Application Form or by other methods to be determined by the Authority. You may send a wet-ink signed copy of this Application Form together with a copy of your ID by registered mail with return receipt requested to the following address:

To ensure the security of your personal data, within seven (7) days from the date your request for information reaches "İstanbul Ekspres Nakliyat ve Ticaret A.Ş. ", the "COMPANY" may contact you to confirm your data ownership and may request certain information and documents from you in this regard. In this context, the information and documents you provide to us will be destroyed immediately following confirmation of your data ownership.

If the requested information and documents are incomplete, we will require that the information and documents be completed and submitted to us upon our request. The thirty (30) day period specified in Article 13/2 of the Personal Data Protection Law for processing the request will be suspended until the information and documents are fully submitted to us.

5. CONCLUDING THE DATA OWNER'S REQUEST

Depending on the nature of your request, we will respond to your request as soon as possible and no later than thirty days after receiving it in accordance with the Personal Data Protection Law. Our responses and evaluations will be communicated to you in writing or electronically, in accordance with Article 13 of the Personal Data Protection Law, depending on your preference indicated in this Application Form. If you have a preference for receiving your application results by mail, email, or fax, please indicate below:

Your requests will be finalized by the "COMPANY" free of charge, however, if the response process incurs additional costs, a fee may be charged in the amounts determined within the framework of the relevant legislation.
